Zoster oticus

Ramsay-Hunt-Syndrom, Ohrzoster, Herpes Zoster des Ohres, Ramsay Hunt syndrome.

Herpes Zoster-Infektion an der Ohrmuschel und Umgebung. Die Hunt`sche Zone umfasst die Auricula, die Membrana tympani und den äusseren Gehörgang. Bei Beteiligung des N. facialis und des N. vestibulocochlearis, Schwindel, Tinnitus und ggf. auch Hörminderung wird vom Ramsay-Hunt-Syndrom gesprochen.

Diagnose durch den HNO-Arzt!

Zu ca. 50-60% kommt es zur Facialislähmung.

• Etwas 1/3 aller Patienten bleiben auf dem Ohr taub
• Manche Quellen empfehlen neben einer antiviralen Therapie eine kurzzeitige Gabe von oralen Glukokortikoiden (Prednisolon 50 mg tgl. p.o.)
• Vorstellung an HNO-Spezialist

• Lichtschutz
• HNO-ärztliche Vorstellung

Topische Therapie

• Je nach Klink unterschiedliche Lokaltherapien.
• Lt. aktuellen Leitlinien wird eine Therapie mit topischen Lokalanästhetika nicht empfohlen.  
• Gerbstoffe Lsg. mehrmals tgl.
• Fusidinsäure Creme mehrmals tgl.
  Fusidinsäure + Betamethason Creme mehrmals tgl.
• Iod-Povidon Lsg. mehrmals tgl.

Systemtherapie

Aciclovir

• Anw.: 10-15 mg/kg KG/d i.v.
• KI: Überempfindlichkeit, Stillzeit (da Übertritt in Muttermilch)
• NW (sehr häufig): Kopfschmerzen, Nausea
• CAVE:
  • Bei älteren Patienten besteht ein erhöhtes Risiko für das Auftreten von reversiblen, neurologischen Störungen
  • Anpassung der Dosierung bei eingeschränkter Nierenfunktion
  • Genügend Flüssigkeitszufuhr

Valaciclovir

• Anw.: 1g 3x/d für 7d
• KI: Überempfindlichkeiten, Stillzeit
• NW (häufig): Kopfschmerzen, Übelkeit
• CAVE:
  • Anpassung der Dosierung bei eingeschränkter Nierenfunktion
  • Hydratationszustand

Famciclovir

• Anw: 500 mg 3x/d für 7 Tage (bei Immunsuppression 10 Tage)
• Von den Mahlzeiten unabhängig
• WW: Probenecid
• KI: Schwangerschaft, Stillzeit, Überempfindlichkeiten gegen Famciclovir oder Penciclovir
• CAVE:
  • Bei Patienten mit dem Risiko einer Dehydratation, speziell bei älteren Patienten, soll auf eine adäquate Flüssigkeitszufuhr geachtet werden
  • Wirksamkeit bei schwarzen Patienten möglicherweise reduziert

Foscarnet

• bei Immunsuppression und Aciclovir-Resistenz
• Anw.: 3 x 40 mg/kg KG/d i.v.
• KI: Überempfindlichkeit, Schwangerschaft, Stillzeit
• NW (sehr häufig): Granulozytopenie, Anorexie, Hypokaliämie, Hypomagnesämie, Hypokalzämie, Parästhesien, Kopfschmerzen, Schwindel, Übelkeit, Erbrechen, Durchfall, Rash, erhöhtes Serumkreatinin, Fieber, Müdigkeit, Frösteln, Asthenie
• CAVE:
  • Anpassung der Dosierung bei eingeschränkter Nierenfunktion
  • genügend Flüssigkeitszufuhr

Prednisolon

• Anw.: 0,5-1,0 mg pro kg KG für 7-10 Tage
