Meningoenzephalitis herpetica

Selten vorkommende, durch Herpes-Viren ausgelöste Gehirnhautentzündung. Häufigste Virus- Enzephalitis bei Erwachsenen.

V.a. bei Neugeborenen und Kindern.

• Akuter Beginn
• Reduzierter AZ
• Schwindel
• Kopfschmerzen
• Meningismus
• hohes Fieber
• Tinnitus aurium
• Übelkeit, Erbrechen
• Krampfanfälle
• komatöser Bewusstseinszustand

• Klinik
• Virusisolation aus Liquor oder Blut
• Procalcitonin ist nicht erhöht (bakt. Meningitis)

Ggl. neurologische Ausfälle.

• Stationäre Aufnahme und Verlegung auf Intensivstation
• Hinzuziehen eines Neurologen/ Infektiologen

• Aciclovir
  • Anw.: 3x 15!mg/kg KG/d i.v.
  • KI: Überempfindlichkeit, Stillzeit
  • NW (sehr häufig): Kopfschmerzen
  • CAVE:
    • Bei älteren Patienten besteht ein erhöhtes Risiko für das Auftreten von reversiblen neurologischen Störungen
    • Anpassung der Dosierung bei eingeschränkter Nierenfunktion
    • Genügend Flüssigkeitszufuhr
